Release of Liability and Photo Release

I understand and assume all risks associated with participating in this activity or event, including but not limited to risks arising from the negligence or carelessness of the organizers or other released parties, from dangerous or defective equipment or property they own, maintain, or control, or from their potential liability without fault.

I confirm that I have no health conditions or medical issues that would prevent me from safely participating in this activity or event.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event organizers for the event in which I am choosing to participate, and that it will govern my actions and responsibilities during the event.

I consent to receive any medical treatment deemed necessary in the event of an injury, accident, or illness during the event.

I understand that photos may be taken during the event and agree that the event organizers may use these photos for any legitimate purpose.
